Crank pulley Bolt, Not the normal questions

Well tonight my Crank pulley and the adjoining bolt decided to take a permanent vacation from my motor, Now my question is, how does a bolt that is torqued down to 190ftlbs wiggle its way out of the crank?

I did recently replace teh timing belt on my civic, say oh 8k ago or so. And i ordered a new bolt from Honda and torqued it down when putting it back on. I am confused as fuck as to what happened. I am seeing this as an excuse to motor swap my car :D. But its just unfortunate that it happened when it did, i got hours cut at work, and i have a shit ton of bills to pay. and no car to get to work now.

was the Woodruff Key installed correctly? It's a little piece of rectangle metal that fits in the slot on the end of the crank & pulley. I had a bolt come loose back in the day & pulley came off when I did not have the key installed.
